Update Your Information
If you are an LDA member and any of your contact information has changed, call the LDA at (225) 926-1986, email info@ladental.org, or fill out the form below.
Once you contact us, your information will be updated in the LDA/ADA membership database.
Note: We also suggest that you make sure your address is correct with the Louisiana State Board of Dentistry. They can be reached at www.lsbd.org or (504) 568-8574. If you have a license through the LSBD, it is required that you keep your address current with them.
